Position: Nurse Practitioner

Location: Bronx, NY.

Duration: 12 months contract with extension possibilities.

Shift: 8:00am - 5:00pm Monday - Friday

Job Duties:

This Nurse Practitioner will support higher levels of complex care at 2 Senior Living Centers in the Bronx. They must be available to float between the 2 facilities.

Primary Responsibilities:

Conduct comprehensive assessments.

Effectively manage acute and chronic medical and behavioral conditions.

Ensure accurate and complete documentation, including ICD 10 conditions.

Communicate and collaborate with the interdisciplinary care team.

Conduct advanced illness and advanced care planning conversations.

Provide patients and caregivers with counseling and education

Required Qualifications: Active and unrestricted license in the state which you reside, or ability to obtain by start date

Education: NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice

Preferred Qualifications: 1 year of clinical experience in practice (long-term care setting preferred)

Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions.

Certifications/Licenses:

• NP NY License
