Pride Health is hiring an RN Case Manager in Port St Lucie, FLI! (Contract)
Traveling to patients home, clinics, and labs in the area
Schedule: Monday-Friday (8am-5pm)
Contract: 3 month contract
Pay Rate: $43-46/hour
Specific Duties:
- Assessment of patients in field
- Phlebotomy
- Collaboration with Supportive Care Team
- Acute Home Visits, Transition Visits, Follow-up Home Visits
- Patient/Family Education
Must have qualifications/experience:
- Field Based Experience as Registered Nurse, preferred
- Florida Registered Nurse License
- BLS Certification
