Pride Health is hiring a Nurse Practitioner in Norfolk, VA! (Contract)

Travel between 3 Long Term Care Facilities (Norfolk, VA and Virginia Beach)

Schedule : Monday-Friday (8am-5pm)

Contract : 3-4 month contract

Pay Rate : $67-69/hour

Specific Duties:

• Provide specialized care to high-risk health plan members
• Work as part of an interdisciplinary team
• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation, including ICD-10 coding
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers

Must have qualifications/experience:

• Nurse Practitioner or Physician Assistant
• NP Certified in one of the following (FNP, ANP, GNP)
• Access to transportation
• 1 year of experience (LTC, SNF, Geriatric preferred)
