Job Summary
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will provide comprehensive care to patients in a primary care setting. The ideal candidate will have a strong background in geriatrics and experience in dementia care, with the ability to perform essential procedures such as suturing and triage.
Duties
- Conduct thorough patient assessments and develop individualized care plans.
- Provide primary care services to patients of all ages, focusing on preventive health measures.
- Manage chronic illnesses and coordinate care for patients.
- Perform diagnostic tests.
- Educate patients and families on health management, lifestyle changes, and health coaching strategies.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
Requirements
- Master’s degree in Nursing from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid state nursing license with prescriptive authority; Active DEA certification a plus.
- Experience in primary care or geriatrics preferred; familiarity with dementia care is a plus.
- Proficient in performing sutures and triage procedures.
- Strong communication skills with the ability to build rapport with patients and families.
- Ability to work collaboratively within a healthcare team environment.
