Care Coordinator LPN/RN - Western Colorado Pediatrics

Job Summary:

The Care Coordinator will be responsible for the care management of a focused group of defined high risk patients under physician’s direction. This person will use motivational interviewing and other professional communication skills to facilitate between the provider and patient/caregivers. They will help empower the patient/caregiver with self-management of their care and assure patients are following their care plan.

Duties and Responsibilities:

  • Ensure patients with complex needs receive proactive, relationship based, care management.
  • Assists patients in the development of self-management goals, evaluate ongoing status of goals along with other members of the interdisciplinary care team; coordinate follow-up as needed.
  • Demonstrates awareness of cultural, ethnic, gender, sexual orientation, socioeconomic and racial differences and their potential to impact patient outcomes.
  • Coordinate the preparation and presentation of patient data on these patients to members of the care team.
  • Knowledge of community resources and services applicable to the pediatric population.
  • Coordinate and manage the care transitions of high risk patients with other medical offices (specialists), health care facilities, and community services that are utilized by the patient.
  • Proactively manage chronic and preventive care for empaneled patients.
    • Use data to continuously improve patient’s health, experience and quality of care, and to decrease cost.
    • Use data to identify populations or groups of patients with similar needs and challenges to select high priority areas for improvement.
  • Conduct regular care team meetings to review quality data and metrics. Use the information to guide tactics to improve patient outcomes and value.

Experience:

  • Two or more years in a medical practice or other health care facility preferred.
  • Experience with documentation in an Electronic Medical Record.
  • Ability to work with computers in word processing, and database applications required.
  • Must be flexible and adapt to a changing environment.
  • Able to prioritize competing responsibilities


Working Conditions:

Works in a well-lighted, ventilated area; standing, sitting, walking, and driving with a varying frequency of moderate lifting.
