Description

We are seeking full-time nurse (LPN/RN) to join our population health team. The nurse will help manage patients as they transition home from facilities and will help to promote wellness activities such as preventive visits and screening services. This is a new role in the department; looking for self-starter who can assist in the development workflows to ensure quality patient care. Must be able to collaborate with clinical providers and team.

Some Job Duties Include:

• Participate in departmental quality improvement activities
• Follow-up with patients following discharge from facilities
• Assist in Quality and gap improvements program
• Collaborate with providers and educate patients
• Care Coordination
• Accurately document all aspects of patient care in the medical chart in a timely manner.
• Annual wellness visits

Requirements

Qualifications & Requirements:

• Graduate from an accredited school of nursing
• Have and maintain current licensure as Nurse in Connecticut
• Prefer a minimum of one (1) year clinic experience

• Bilingual preferred - Spanish
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills
• Ability to work independently
