Prime Home Health and Pacific Hospice & Palliative Care is Hiring a Physical Therapy Assistant - Home Health Near Dallas, TX

Prime Home Health is rapidly growing and seeking several dedicated and compassionate Physical Therapy Assistants to join our home health care team on a PRN basis in the Dallas and Fort Worth areas. As a PTA, you will provide valuable support to our physical therapists by assisting in the implementation of individualized treatment plans and delivering high-quality care to patients in the comfort of their Dallas area homes. 

Your primary responsibility will be to help patients regain mobility, strength, and independence, contributing to their overall well-being and improving their quality of life.

Responsibilities:

  1. Assist the physical therapist in the evaluation and assessment of patients' conditions, including reviewing medical history and conducting relevant tests and measurements.
  2. Collaborate with the physical therapist to develop individualized treatment plans based on patients' goals, needs, and diagnoses.
  3. Implement treatment plans under the supervision and guidance of the physical therapist, which may include therapeutic exercises, functional training, gait training, and other modalities.
  4. Monitor and document patients' progress, adjusting treatment plans as necessary and reporting any changes or concerns to the physical therapist.
  5. Educate patients and their families on exercises, activities, and strategies to maximize function, prevent further injury, and promote independence.
  6. Provide assistance to patients during therapy sessions, ensuring their safety and comfort.
  7. Help patients with activities of daily living (ADLs) and mobility exercises, including transferring, ambulation, and using assistive devices.
  8. Maintain accurate and up-to-date patient records, documenting treatment sessions, progress notes, and any relevant information in accordance with regulatory requirements.
  9. Adhere to ethical and professional standards, maintaining patient confidentiality and respecting their rights and dignity at all times.
  10. Collaborate effectively with the interdisciplinary home health care team, including physicians, nurses, occupational therapists, and speech-language pathologists, to ensure comprehensive patient care.
  11. Participate in professional development activities, such as continuing education courses and conferences, to stay updated on the latest advancements in physical therapy.

Qualifications:

  1. Minimum 1 year in an Adult In-Home Health care setting is required. 
  2. Associates degree from an accredited Physical Therapy Assistant program.
  3. Valid State of Texas license or certification as a Physical Therapy Assistant.
  4. Proficiency Kinnser/Wellsky EHR preferred, other relevant software such as Slack. 
  5. Current CPR BLS certification
  6. Solid understanding of physical therapy principles, techniques, and modalities.
  7. Excellent interpersonal and communication skills to effectively interact with our scheduling team, patients, their families, and the multidisciplinary team.
  8. Strong observational and problem-solving abilities.
  9. Ability to work independently while following established treatment plans and protocols.
  10. Physical stamina and dexterity to assist patients with mobility exercises and transfers.
  11. A commitment to providing compassionate care and maintaining professional ethics.
