Responsibilities:
- Provide direct patient care, including health coaching and education
- Assist with discharge planning and coordination of care
- Perform basic nursing procedures such as medication administration, and vital signs monitoring
- Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans
- Utilize electronic health record systems
- Assist with diagnostic procedures, such as sonography and ophthalmology exams
- Provide behavioral health support to patients
Skills:
- Valid license as a Licensed Vocational Nurse (LVN)
- Strong communication and interpersonal skills
- Ability to lift and move patients safely
- Knowledge of medical terminology and procedures
- Familiarity with electronic health record systems
