As a Physician at Landmark, you will lead a dedicated multidisciplinary team and deliver comprehensive medical care to complex and vulnerable patients right in their homes.
Job ResponsibilitiesClinical Leadership:
  • Provide daily decision support to nurse practitioners and lead the multidisciplinary team.
Preventive And Urgent Care
  • Conduct 4-6 preventive visits daily to optimize chronic conditions, assess home environments, educate patients and caregivers, and create proactive care plans.
  • Perform urgent care visits in patients' homes to prevent unnecessary emergency department transfers and hospital admissions.
Collaborative Care
  • Collaborate with nurse care managers, behavioral health specialists, social workers, pharmacists, and dieticians to address patients' medical, biopsychosocial, and financial needs.
  • Serve as the primary care provider (PCP) for patients without an existing PCP, ensuring continuity of care.
  • Co-manage patients with existing PCPs to extend clinical care into the home environment.
  • Coordinate with physicians across different care settings, including PCPs, hospitalists, and skilled nursing facility providers, to facilitate seamless transitions and reduce readmissions.
  • Offer medical guidance to community-based organizations involved in our patients' care, such as housing agencies, caregiver services, and behavioral health providers.
Qualifications
  • Board Certification, preferably in Internal Medicine, Family Medicine, or Emergency Medicine.
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution.
  • DEA registration.
  • Experience managing complex, chronically ill patients with high acuity.
