This is a full-time on-site role for Home Healthcare and Hospice Clinicians at Professional Healthcare Resources in Annandale, VA. Clinicians will be responsible for providing skilled nursing care, therapy, personal care, and hospice services to patients, helping them recover from hospitalizations, manage illnesses, or assist with daily activities. Qualifications

• Skilled Nursing, Therapy, and Personal Care skills
• Knowledge of Hospice care and end-of-life support
• Strong assessment and patient care planning abilities
• Effective communication and interpersonal skills
• Empathy, compassion, and respect for patients
• Ability to work collaboratively in a multidisciplinary team
• Current RN, LPN, PT, OT, or ST license in Virginia
• Experience in Home Healthcare or Hospice is a plus
