PROJECT AMISTAD is Hiring a Guardianship Case Manager Near El Paso, TX

TITLE: Guardian Case ManagerSUPERVISOR: Guardianship Program ManagerSTATUS: EXEMPT The incumbent functions as a Guardianship Case Manager of wards assigned by the Probate Courts of the County of El Paso. The incumbent shall make responsible decisions and use advanced judgment to ensure that decisions made are in the best interest for the ward’s health and welfare. The incumbent advocates for the ward and ensures that the basic needs to include food, shelter, and medical treatment are met. The incumbent will also secure any benefits that the ward is entitled to. Timely and accurate documentation is required and essential in recording the services provided to the ward. Work requires tolerance and sensitivity to the needs of the wards. This position requires that the incumbent is on call for their assigned caseload on a 24-7 basis to include weekends and holidays. The incumbent is also required to use their own vehicle to complete monthly visits, appointments and meetings and transport clients to their appointments, medical or otherwise in the agency provided vehicles, etc. The Case Manager may be assigned to administer the duties of an estate or a guardianship of a person of diminished capacity. The work environment is fast paced, with time sensitive issues and individuals in distress or in extremely difficult situations. The incumbent is expected to handle sensitive situations, maintain a professional demeanor, and a positive attitude in a high-pressure environment.Knowledge, Skills, and Abilities
  1. Conducts guardianship assessments, prepares all documentation necessary to present to the County Probate Court and testifiesat court hearings in order that the Court make an informed decision regarding the guardianship of an individual.
  2. Executes the orders of the Probate Court or requirements as specified by the County Probate Court, or other entities.
  3. Implements all contractual requirements and completes all required documentation in a timely manner to include documentation of all case events and annual reports.
  4. Makes and carries out living arrangements that are in the best interest of wards. This may include removal from current residence to another domicile such as a group home, nursing home or more structured facility. In all instances, the least restrictive alternative will be first option to be considered.
  5. Monitors the living condition of assigned wards to ensure that he/she is receiving care, comfort and supervision, and that all of their needs are met, whether the ward resides alone, in a group home, a nursing home or in a more restrictive facility.
  6. Advocate for ward’s needs and monitor the continuity of care/services.
  7. Drive themselves in their own vehicle to conduct home and/or quality assurance visits, as well as driving themselves to attend doctor appointments, meetings and any other visits deemed necessary for the wellbeing of the ward.
  8. Transport clients to their appointments, medical or otherwise in the agency provided vehicles, etc.
  9. Secure and inventory ward’s property, and coordinate sale, abandonment/donation, of property as specified in procedural manual and contract specifications.
  10. Secure any benefits/pension the ward is entitled to, coordinate a budget, and monitor accounts to avoid impropriety.
  11. Upon the death of a ward, ensures that all funeral arrangements are made, that all documents related to the death of a ward are completed and filed appropriately, that known family members and all parties involved in the funeral process are notified, and ensures that the deceased ward is buried or cremated as timely as possible.
  12. Markets and promotes Amistad by making presentations about the agency to other agencies, organizations, community fairs, or through media outlets.
  13. Becomes familiar with the agency’s procedural manuals, personnel manual, training materials, contractual requirements, and internal forms necessary to meet the needs of the Guardian Case manager position.
  14. Completes other duties as assigned by the Guardianship Program Manager, COO of Social Services and/or Chief Executive Officer.
Essential Requirements1. BA degree and experience in area of delivery of human services.2. Demonstrate ability to complete written reports and make oral presentations to small or large groups.3. Must have a current driver’s license and show evidence of meeting state requirements of having the appropriate level of automobile insurance and drive themselves or clients to appointments, meetings, etc.4. Able to lift and carry up to 40 pounds.5. Must agree to undergo a background check by law enforcement as well as a drug test. Results must show that person has not been convicted of an offense nor have a positive drug test result.6. Successfully complete the Guardianship Certification Exam through the Judicial Branch Certification Commission (JBCC) within one year of employment. The employee will be required to take the exam at the next available date after on-boarding and after on the scheduled dates within one year if they unsuccessfully pass the exam.7. Bilingual - must demonstrate the ability to communicate in Spanish & English (written & verbal).8. Able to attend training sessions within and outside assigned region and drive themselves to provide services to ward’s place of residence.In addition to the Agency’s background check, JBCC will conduct its own background check to include fingerprints prior to issuing a Guardianship Certification. All applicants for a guardianship position must receive that certification and recertify for the certification every two years. The JBCC considers the following when conducting a criminal history check of a Guardianship Certification applicant: • Crimes against the person such as assaultive offenses, kidnapping, and murder. • Crimes involving prohibited sexual conduct or involving children, the elderly, or disabled persons as victims. • Crimes involving dishonesty, fraud, misrepresentation, deceptive business practices, or misapplication of fiduciary property. • Crimes involving tampering with government documents. • Crimes against property such as theft or burglary. • Crimes involving alcohol and drug-related offenses. • Crimes involving deliberate violence.This job description should not be construed to imply that these requirements are the only duties, responsibilities and qualifications for this job. As with all funded programs, the position is contingent upon continuation of funding. Evaluation: A review of performance is conducted after a four-month introductory period, and upon completion of twelve months from the date of hire, and every twelve months thereafter.
