Promise Regional Medical Center Hutchinson is Hiring an OCK Case Manager - FT - Community Support Services - Horizons Near Reno, KS

Provides comprehensive case management to consumers who have opted in to the One Care Kansas Medicaid program. This provider will help coordinate each consumer’s overall healthcare with all healthcare providers and will collaborate with each of them to ensure that the care provided takes each piece of the consumers health into consideration. The OCK Case Manager’s work will be largely driven by the consumer’s Health Action Plan and will directly correlate to the short-term and long-term goals that the consumer develops. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. ESSENTIAL FUNCTIONS: Helping consumers develop short term and long-term goals related to their overall health. Completing a Health Action Plan with each consumer every 90 days. Educating consumers about health practices and helping motivate consumers to seek better health outcomes Helping consumers access various forms of healthcare depending on their need Helping consumers establish routine healthcare practices (dental visits, vision appointments, primary care, labs, medication management, specialist visits, etc…). Referring consumers to various community resources that can help in improving overall health. Helping consumers develop positive social and familial supports Helping coordinate successful transition plans from one level of care to another (i.e. inpatient hospitalization transitioning to independent living). Performing routine assessments with consumers to evaluate their level of need and what services might be most beneficial to them. Other duties as assigned. MINIMUM KNOWLEDGE AND SKILLS REQUIRED: Proficient in electronic documentation, good computer proficiency Experience working with educating and teaching clients about self care, improving daily living skills Strong organizational skills Good time management skills Be an active listener, positive mentor/role model REQUIRED BEHAVIORAL SKILLS: Integrity: A personal presence which is characterized by a sense of honesty and the willingness to do the right thing. PREFERRED BEHAVIORAL SKILLS: Working knowledge of various service delivery systems and an ability to help consumers navigate those systems. Knowledge of information sharing between health agencies Knowledge of basic health practices and the ability to educate consumers about ways to improve their overall health. MINIMUM EDUCATION AND EXPERIENCE REQUIRED: One of the following is required: Bachelor’s degree, preferably in Social Work, Psychology, or some related field OR Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities OR Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years. At least one year working on some health-related field. PREFERRED EDUCATION AND EXPERIENCE: Degree in Psychology, Social Work or other Human Services field. Three to five years’ professional experience in health services or related field.
