Job SummaryWe are seeking a compassionate and dedicated Behavioral Health Specialist to join our team as a Case Manager in Columbus, OH. The successful candidate will provide support and case management services to individuals experiencing behavioral health challenges. You will work collaboratively with clients, their families, and other health professionals to ensure that clients receive comprehensive care and support.
Key Responsibilities
- Conduct initial assessments to determine clients' needs and develop individualized service plans.
- Provide ongoing case management and support to clients, including regular follow-ups and adjustments to service plans as needed.
- Coordinate with healthcare providers, social services, and community resources to ensure clients receive necessary services.
- Advocate for clients to help them access resources and navigate the healthcare and social services systems.
- Monitor and document clients' progress, maintaining accurate and up-to-date case files.
- Develop and facilitate group sessions or educational workshops as needed.
- Assist clients with setting and achieving personal goals related to their health and well-being.
- Ensure compliance with all legal, ethical, and organizational standards and guidelines.
- Attend regular team meetings, training sessions, and professional development opportunities.
- High school diploma or equivalent.
- Minimum of 3 years of experience in a related field, preferably in behavioral health, social work, or case management.
- Valid driver's license and reliable transportation.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational and time management skills.
- Compassionate, patient, and empathetic approach to client care.
- Basic computer skills, including proficiency with Microsoft Office Suite.
- Certification in behavioral health or case management is a plus.
- Experience working with diverse populations, including individuals with mental health and substance use disorders.
