Required Qualifications:
* Associate's Degree in Physical Therapy, from an accredited program.
* National Provider BLS - American Heart Association within 30 days of hire.
* Oregon Physical Therapy Assistant License upon hire.
* Washington Physical Therapist Assistant License upon hire - if practicing within this state.
