DescriptionThe Community Health Worker (CHW) promotes comprehensive health within the community they reside in. CHWs are an integral member of patient-centered health teams. They walk along side neighborhood residents to achieve and maintain their ideal state of physical and mental health through prevention, education, and access to appropriate treatment. The role of the community health worker is complex and will vary in response to the needs of individual neighborhoods.
