Position: Direct Support Counselor

Position Description:  The Direct Support Counselor is responsible for the daily support and oversight of clients living in the Intensive Residential Support Home. This position provides care for individuals living with disability and/or mental health illness by providing support and assisting with daily activities in their homes. This will include working on identified goals such as meal cooking and preparation, personal cares, promoting community involvement, social skills etc. You will be responsible for administering medications during the shift in needed. The home operates 24 hours a day. THIS POSITION IS OPERATIONAL 24/7. WEEKENDS AND HOLIDAYS ARE REQUIRED. We have shifts on 1^st, 2^nd and 3^rd .

1^st shift : 7:00 am to 3:00pm

2^nd shift: 3:00pm to 11:00pm

3^rd shift 11:00pm to 7:00 am. These shifts are subject to change based on client need.

Job Description:

1.Responsible for treating all clients with dignity, respect, and value.

2.Responsible for the health, safety, protective supervision, and wellbeing of clients on your shift when working in the home.

3.Responsible to immediately report any allegations of client abuse to the Program Manager.

4.Responsible to complete all required trainings and orientations within the period specified by the Program Manager.

5.Responsible to complete all required documentation on your shift before going home, including, but not limited to, service documentation, data sheets, incident reports, and medication administration records.

6.Responsible to provide support to clients with implementation of their personal goals and objectives.

7.Responsible to provide supervision and assistance to clients with informal supports they may need help with, including but not limited to, cooking, cleaning, laundry, planning meals, shopping, and personal hygiene.

8.Responsible to assist clients with activities of daily living/personal hygiene that they absolutely are unable to do for themselves.

9.If certified to administer medications, you are responsible for the timely safe administration and documentation of medications in the MAR.

10.Responsible to maintain the confidentiality and privacy of client information.

11.Responsible to implement client behavior intervention plans when their problem behavior warrants.

12.Promptly report any unsafe conditions in the home to the Program Manager.

13.Reports changes in client’s condition or family situation to the Program Manager.

14.Accompanies clients to scheduled appointments and transports clients or running errands with or for clients.

15.Other reasonable duties as requested by the Program Manager.

Other Functions and Key Responsibilities

If certified to administer medications, will need to administer medications when working.

If not certified to administer medications, will need to secure the certification.

Will need to secure CPI/Mandt Training

Must have a valid driver’s license and car insurance

Qualifications/Skills:

At least a high school education or equivalent

At least one years’ experience working with individuals with a mental illness or multi-occurring conditions is required.

Medication manager or certified medication aide preferred (will be required to be trained in medication manager)

1 year Experience supporting individuals who experience problem behaviors is required.
