POSITION SUMMARY

• The mission of the Case Manager is to provide services and supports necessary to assist persons served in achieving and maintaining rehabilitative, sobriety, and recovery goals. This service is intended to increase skills to address the complex mental health and/or substance abuse needs of adults who have significant functional deficits in order to promote symptom reduction; assist recipients in acquiring mental health/substance abuse recovery skills necessary to successfully address vocational, housing, and educational needs; assist recipients in gaining access to and coordinating necessary services to promote clinical stability, and meet their mental health/substance abuse treatment, social, and other treatment support needs. 
• The Case Manager assists in obtaining entitlements, resources, housing, educational opportunities, employment, etc. to enable the person to become as independent as possible

EDUCATION & EXPERIENCE:

Completion of a higher education degree in psychology, counseling, social work, rehabilitation, behavioral science. Experience with persons with serious mental illness, homeless support, assertive community treatment or a similar community support setting is preferred.
