PUBLIC HEALTH MANAGEMENT is Hiring an Anti-Violence Case Manager/Coordinator Near Philadelphia, PA

JOB DESCRIPTION:

BIPS is looking for an enthusiastic, energetic self-starter with a passion for preventing gun violence. The ideal candidate will have a BA or higher (MA preferred) in a human services field or equivalent experience. The ideal candidate should have proven community organizing abilities. He or she will have excellent communication skills (written and verbal). The ideal candidate would be equally at home conversing with gang members, community activists, law enforcement personnel, and public officials.

Responsibilities:

  • Work and support with young people who have been affected by some form of violence (gun/physical).
  • Provide intensive case management, mentoring and advocacy to highest risk individuals involved in violence, youth involved in Juvenile Justice System, and school-based services.
  • Works to prevent gun violence by 1) mediating conflicts that may end in gun violence, 2) changing the thinking of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with local partners to promote a visible community-wide message that shooting is unacceptable behavior.
  • Develop and maintain relationships with local service providers and program partners, including e.g. law enforcement, faith leaders, and community stakeholders, in order to identify and access resources for high risk individuals.
  • Create and implement a community mobilization plan that encourages resident involvement in activities designed to change local norms towards gun violence.
  • Organize and execute regular outreach community events, including responses to shootings in the target area; Participate in violence prevention efforts with other providers as assigned.
  • Maintain intensive follow-up contact with clients, family, friends and service providers through home visits and telephone contact; Provide support to family and friends of youth.
  • Provide client referrals to community service providers.
  • Assist and/or co-facilitate youth support groups.
  • Help to develop the Bridge anti-violence social media presence; Develop and disseminate anti-violence public education materials
  • Assist with monitoring of youth on program site.
  • Participate in Service Plan meetings as warranted.
  • Participate in clinical team meetings, staff meetings and trainings, as instructed.
  • Maintain accurate up-to-date case notes. Input notes in the CBPS database and document consistently and accurately in records all contacts with clients.
  • Assist with administrative duties as needed.
  • Other duties or activities as assigned by the Program Director.

Qualifications

  • Bachelor’s Degree preferred with emphasis in Social Work or related field (but not necessary). An equivalent combination of education and experience may be considered.
  • At least two years experience working with at-risk youth and experience or training in crisis intervention. Must have valid PA driver’s license, insurance, access to a vehicle, and good driving record. An equivalent combination of education and experience may be considered.
  • Excellent communication skills (written and verbal)
  • Ability to navigate relationships with people of diverse faiths, ages, races, gender identities, etc.
  • Proven community organizing abilities
  • Extensive knowledge of Philadelphia and other high-risk neighborhoods
  • Excellent interpersonal and organizational skills
  • Working knowledge of all applicable computer software
  • Comfort with traveling and working directly at the community level in areas where violence is active and during peak hours when conflicts and shootings are likely.
  • This position requires a background check.
  • This position requires Child Abuse Certifications prior to the commencement of service.
  • Demonstrated commitment to working with youth and young adults.
  • Knowledge of urban youth issues, specifically youth violence;
  • Demonstrated ability to work independently and as part of a team;
  • Must have strong communication and organizational skills, and possess the ability to communicate effectively with staff members, clients, vendors and the public;
  • Ability to work well with diverse populations;
  • Sensitivity to racial, cultural, economic, and willingness to adopt interventions that work for different groups.
  • Understanding of a strengths-based, recovery-oriented approach and a willingness to adopt this approach.
  • Knowledge of high-risk youth, family systems, mental health disorders and substance abuse.
  • Ability to work independently.
  • Ability to navigate throughout the city comfortably and effectively.
  • Comfortable working with adolescent youth (ages 10 to 19).

Experience:

  • Case management and service delivery experience (with a minimum of three years of experience preferred)
  • Experience working with justice-involved populations, demonstrated knowledge of challenges facing justice-involved populations
  • Knowledge of and experience working with Philadelphia communities
  • Knowledge, understanding and experience working with youth, with preferences given to having worked with at-risk youth, substance abuse, trauma and/or mental illness
  • Knowledge and understanding of Philadelphia’s social service system.
  • Knowledge of neighborhood and community resources relevant to this population.

Education:

  • Bachelor’s Degree preferred with emphasis in Social Work or related field
