JOB DESCRIPTION:

Full Time Bi-lingual FIR Case Manager reports to the FIR case management supervisor. This individual works directly with participants that have Forensic Service involvement and may have oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to CBH, Forensic Services and PHMC requirements. This individual will provide support to the participants while working with all levels of management within the treatment program, Forensic Services, and recovery community.

Responsibilities:

  • Productivity requirement to obtain 83 billable services per week for active case load. This equates to 332 target units per month. Months with less than 4 weeks monthly target varies.
  • Provide case management services to chemically dependent/and or co-occurring individuals with a caseload of active 25 participants. Primary case load will consist of a mix of bi-lingual Spanish speaking individuals.
  • Establish and maintain service productivity goal at minimum designated service units per week.
  • Documentation of service provision within 24 hours of service completion
  • Active participation in team meetings, scheduled case conferences, trainings as assigned.
  • Active participation in individual supervisions, case reviews, and schedule team building events as assigned.
  • Collaboration with identified court personnel, probation/parole, D&A treatment providers and other assigned agencies as identified.
  • Completion of discharge summaries within timeframes specified by program
  • Assist individuals to link to recovery support services including Pro-ACT recovery center, Access to Recovery, faith-based institutions and other identified community supports
  • Weekly schedules with detail activities updated daily in outlook feature
  • Maintain participant records in accordance with agency fee for service standards, managed care, and licensing requirements
  • Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services.
  • Identify, coordinate and link community resources to participant to promote accessibility to recovery-oriented community services
  • Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach.
  • Average of 95% work week in the community for participant appointments and provider meetings.
  • Flexible work schedules available with supervisor approval if needed to provide participant services. Adherence to documentation and approval guidelines required.

SKILLS:

  • Valid driver’s license
  • Good customer service skills
  • Ability to work remotely in the community accessing public transportation
  • Good time management skills
  • Excellent oral/written communication skills with the ability to type
  • Good analytical and problem-solving skills
  • Must be able to speak fluently in Spanish.

EXPERIENCE:

  • Experience in direct case management services with the chemically dependent criminal justice population a plus. Experience in fee for service delivery a plus with experience in writing service notes.

EDUCATION:

  • Minimum an Associates’ degree in social services, sociology, psychology, criminal justice or social work and 2 years’ experience in public and private human services with 1 year in direct participant contact in addiction recovery, mental health and behavioral health setting.
  • Bachelor’s degree preferred with major course work in sociology, social work, psychology, anthropology, political science, history, criminal justice, counseling or education and at least one (1) year of direct participant contact in addiction recovery, mental health and behavioral health setting.
