PUBLIC HEALTH MANAGEMENT is Hiring a Community Liaison/Case Manager Near Philadelphia, PA

Job Overview:

Well versed, organized, and personable individual needed to serve as an case manager for JJPI’s CIP programs (Maternal and Family Mental Health Collaboration and Delco HART). This position will work under the supervision of the Special Projects, Prevention and Outreach Manager and Director of Survivor Services.

DELCO HART RESPONSIBILITIES:

  • Help screen referrals and collaborate with clinicians to complete participant intakes.
  • Maintain contact with participant families and provide case management services over time to better ensure that their needs continue to be identified and appropriately addressed.
  • Identify community resources and facilitate participant referrals.
  • Arrange for transportation, interpreters, and other add-on services necessary to address barriers to DelcoHART services.
  • Participate in team meetings related to client needs, services, and system involvement.
  • Assist in the collection of paperwork related to informed consent, demographics, clinical assessment, and program evaluation.
  • Advocate on behalf of participants and their families.
  • Participate in staff meetings at all levels of the agency, as appropriate or as assigned.
  • Log activities, maintain program statistics, update databases, help maintain clinical records, and prepare related reports.
  • Complete and submit all required, relevant, and assigned paperwork in a timely and accurate manner.

Maternal Mental Health Consortium Responsibilities:

  • Help screen referrals and collaborate with clinicians to complete participant intakes
  • Collaboratively update operational or promotional materials including and not limited to program manuals, training materials, forms, and graphics.
  • Serve as a liaison between the Mable Morris, Nurse Family Partnership, and National Nurse Care Consortium teams and the Maternal Mental Health Collaboration staff.
  • Lead, coordinate, and facilitate internal and external meetings including but not limited to maternal mental health collaboratives and maternal and child health workgroups.
  • Contribute to the development and enhancement of a centralized intake system to improve maternal and family mental health coordination and referrals.
  • Assist in the development, scheduling, and facilitation of professional development trainings
  • Assist in the composition and submission of requested reports and assignments by the announced deadline.
  • Maintain understanding of current implementation challenges and develop solutions to address them.
  • Maintain compliance with guidelines and requirement of various trainings.
  • Log activities, maintain program statistics, update databases, help maintain clinical records, and prepare related reports.

Skills:

  • Must possess excellent interpersonal, communication, and writing skills
  • Must effectively function as part of a multidisciplinary team consisting of executive staff, nursing professionals, behavioral health professionals, and community-based staff
  • Must have experience with program coordination and program management
  • Must have extensive experience using Microsoft Office (Word, Excel, Outlook, etc.)
  • Must have a strong positive orientation for connecting with and finding supports for trauma survivors

Experience:

  • 2 years of Public Health training and/or practice in a professional or academic setting
  • Social Work and clinical knowledge is a plus.

Education Requirement:

  • Bachelor’s degree in public health, social service, or any related field
