Job Description

The Nursing Supervisor – Adult Shelters will supervise four community health nurses in adult shelters and coordinate with one psychiatric nurse practitioner providing services to adults in male and female shelters, café’s, and multiple street outreach locations. All services are provided in collaboration with on-site or community-based case managers and with any other HCH staff providing services to the target population. The Supervisor is a member of the Emergency Management Team through the City of Philadelphia and is part of the emergency preparedness leadership team. The Nursing Supervisor provides education in collaboration with the Department of Behavioral Health as well as The Department of Public Health. In addition, the nursing supervisor may provide direct consultation services to adults in her area of specialty.

RESPONSIBILITIES:

Supervision and Management

  • Participate in management of team in coordination with Nursing Supervisor – Family Shelters, Clinical Director – Health Network, and Director of Homeless and Social Service Programs,
  • Provide Infection Control in the Adult Shelters in coordination with the Department of Public Health and the Office of Homeless Services.
  • Ensure services are provided in accordance with VFC/VFAAR, Joint Commission, Department of Public Health, and Office of Homeless Services regulations.
  • Orient, supervise, and review performance of staff providing triage of acute health problems (medical and behavioral health), outcomes-oriented family planning and chronic disease case management, adult immunizations and other health services in shelters.
  • Conducts a minimum of monthly site monitoring visits.
  • Conducts monthly infection control observation monitoring of family shelter nurses.
  • Develop, mentor and maintain on-site HCH teams designed to achieve clinical outcomes.
  • Oversee tracking of follow-up, HIPAA compliance, medication safety, environment of care and other risk-management activities at clinical sites.
  • Oversee lab services, medical waste pick-up, manage shelter supplies, and other shelter-based services as applies to appropriate participating sites.
  • Reviews, records and submits monthly CLIA lab reports.
  • Reviews, records and submits monthly AACO reporting measures.
  • Oversees seasonal influenza vaccine process in collaboration with site nurse(s)
  • Maintains site sign requirements to meet Joint Commission requirements
  • Monitors site equipment maintenance.
  • Attends UDS measures meetings as requested.
  • Assists with trouble-shooting Electronic Medical Record issues
  • Conducts auditing of nursing documentation; provides feedback and corrections.
  • Assists in obtaining specialized support through Managed Care Companies for shelter clients.
  • Routinely monitor and report outcomes based on Family Planning Council reports, HCH data, on-site logs, chart review and develop action plans to address issues identified.
  • Develop collaborative relationships with shelter providers and community health care providers including the Family Planning Council, Department of Behavioral Health, Medical Examiner Office, AACO, VFAAR/VFC as needed for consultation, Department of Public Health, Office of Homeless Services, and with Managed Care Companies.
  • Monitors current UDS measures and implements new UDS measures throughout all free-standing adult sites
  • Assume new responsibilities as new needs are identified.

Infection Control/Emergency Preparedness

  • Provides coordination and monitoring services for report of suspected or confirmed communicable diseases that can be transmitted through routine shelter contact. Provides twenty-four hour on-call services for all adult shelter services through the City of Philadelphia Office of Homeless Services and their affiliates.
  • Monitor shelter sites for consistent hand washing, care of environment, use of safety syringes and sharps disposal, use of personal protective equipment.
  • Manage or participate, as needed, in shelter based communicable disease outbreaks, emergency preparedness / disaster response and other health initiatives.
  • Train community and academic health care professionals to provide comprehensive, culturally competent, continuous care to homeless people.
  • Provides monthly tracking and record keeping on-site for environment of care report monthly. Discuss any deficiencies with Director of Homeless and Social Service Programs and monitor remediation of deficiencies.

Clinical

  • Provide on-call /on-site consultation for shelter residents adjunct to community health nursing services as needed (e.g. no nurse available)
  • Provides resources for screening and immunization to shelter staff, as indicated
  • Document all services in a timely manner
  • Advocate for consumers and HCH program, as appropriate.
  • Participate in advocacy and coordination committees addressing issues related to homelessness
  • Provides clinical data review for Philadelphia Medical Examiner’s Office.
  • Provides infection control support throughout City of Philadelphia Adult Shelter sites
  • Collaborates with DPH, OHS, and shelters for timely resolution of infection control measures
  • Participates in on-going infection control training through in-person and webinar formats

Experience:

  • Five years in nursing field
  • Experience in community health nursing, preferred
  • Experience with providing care in shelter and safe haven environments preferred
  • Infection control experience
  • Emergency preparedness experience
  • Supervisory experience required
  • Refined organizational skills and leadership qualities
  • Experience in health care planning and administration within an ambulatory health setting serving a multi- problem clientele

Education:

  • BSN or MSN preferred
  • Licenses and certifications: Pennsylvania RN

Physical Demands:

Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting under 1/3 of the time, use of hands to finger, handle or feel 2/3 of the time, reach with hands and arms, under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs. up to 1/3 of the time.

  • Travel is required between clinical site(s) by personal automobile or public transportation, reimbursed in accordance with PHMC’s travel reimbursement policy.

Work Environment:

Moderate noise (examples: business office with computers and printers, light traffic). Exposure to blood borne pathogens that requires use of personal protective equipment

Primary work environment is shelters, considered high risk for tuberculosis and other air-borne diseases. Communicable diarrheal diseases are common, particularly in family shelters, but transmission can be interrupted by appropriate hand washing. Many shelters are not wheelchair accessible.

IMPACT OF ERROR:

Clinical errors in this position, including poor clinical judgment and missed opportunities can result in serious consequences including injury, illness, and death. Measures to reduce the probability of clinical errors or their consequences include:

  • Credentialing process
  • Orientation program
  • Training in CPR and use of emergency equipment
  • Supervision/review of clinical records
  • Tracking and recall systems
  • Verbal and written information to clients
  • Self audits
  • Chart and MIS audits through Continuing Quality Improvement Committee
  • Audits by outside agencies, e.g. Family Planning Council, PCER
  • Employee health policy regarding screening, immunizations, and injuries including needle sticks
  • Client satisfaction surveys and focus groups

Procedural errors can result in loss of reimbursement for clinical services or loss of funding streams. Measures to reduce the probability of procedural errors include:

  • Training on documentation and record keeping
  • Supervision/review of electronic health record
  • Following established processes
  • Tracking and recall systems
  • Chart and IT audits through Continuing Quality Improvement Committee
  • Audits by outside agencies, e.g. Family Planning Council, PCER, JCAHO, HRSA
  • Attends CQI Committee meetings when appropriate

Communication/collaboration errors can result in loss of credibility with consumers, other agencies, and funders. Measures to reduce the probability of errors include:

  • Regular and ad hoc meetings with collaborating agencies
  • Clinical workgroups
  • Management workgroups
