Essential Duties
- Provide end-of-life care to patients in their homes
- Monitor and address pain and symptom management
- Provide skilled observation, assessment, and evaluation
- Provide patient and family/caregiver support and education
- Practice cohesive and collaborative care with the healthcare providers
- Supervise LPN and CNA staff
Requirements and Qualifications
- Registered Nurse (RN) licensure
- Prior Home Health and/or Hospice experience strongly preferred and wage is based on hospice experience
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Be a licensed driver with an insured automobile in good working order
