Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide comprehensive nursing care to patients and coordinate their overall care plan.
Duties:
- Conduct patient assessments and develop individualized care plans
- Coordinate with healthcare providers to ensure proper patient care
- Monitor patient progress and update care plans as needed
- Maintain accurate medical records and documentation
- Provide patient education on treatment plans and medications
- Collaborate with interdisciplinary teams to optimize patient outcomes
Requirements:
- Associates or Bachelor's degree in Nursing (BSN) required; Master's degree is a plus
- Valid RN license in the state of practice
- Minimum of 2 years of nursing experience in home health, acute rehab, long term care or alf settings
- Strong knowledge of HIPAA regulations and patient confidentiality
- Excellent critical thinking and communication skills
- Experience in hospital or critical care settings preferred
