- V510800 Experience in any one (or a combination) of the following: Athletic Trainer Certified, Chiropractor, Dentist, Dental Hygienist, Registered Dental Assistant, Licensed Practical Nurse, Respiratory Care Practitioner, Physical or Occupational Therapist, Physical or Occupational Therapy Assistant, Pharmacist, Medical Physician, Physician’s Assistant Certified Registered Nurse, Advanced Registered Nurse Practitioner.
- V510709 Experience in one of the following: Athletic Trainer Certified, Chiropractor, Physical or Occupational Therapist, Physical or Occupational Therapy Assistant.
- Must hold or be willing to obtain CPR/First Aid Certification and OSHA Trainer Certification
- Pass Criminal Background Fingerprinting with WSP and FBI
- Successfully complete OSPI required training
