QSAC, Inc. is Hiring an Assistant Coordinator - Family Support Services Near New York, NY

RESPONSIBILITIES

Complete required QSAC orientation.
Responsible for assessment, development and coordination of services for individuals and families in accordance with the OPWDD FSS Contract for Non-Medicaid Service Coordination.
Assess the needs of the individual and develops an effective plan of service with the individual and parent.
Make ongoing referrals as needed and assists in linking the individual to those services.
Monitors existing services to ensure appropriateness and ongoing need.
Coordinate the delivery of services with individual/family, other agencies as well as other involved parties (i.e., schools, day programs, etc.).
Coordinate and manage day to day operations within the Family Reimbursement programs. (FSS/Rent Subsidy)
Manage and facilitate regular Self-Advocacy groups.
Crisis intervention and follow-up as needed.
Maintain individual files and adhere to confidentiality requirements.
Attend trainings in accordance with OPWDD and QSAC requirements.
Punctuality and regular attendance is expected.
Commitment to company values and adherence to policies.
Perform other duties as assigned by supervisors and/or senior management.

Salary Range - 40,000-42,000

MINIMUM QUALIFICATIONS

Masters Degree in Health or Human Services OR
Associate’s Degree in Health or Human Services, or an RN. An individual may meet this
education requirement in one of two alternative ways:
1. A candidate for a Bachelor’s Degree may meet this educational requirement by providing a letter from
his/her college verifying that the candidate has completed course work equivalent to an Associate’s Degree in Health or Human Services.
2. An individual with an Associate’s Degree or a Bachelor’s Degree or who has a minimum of 60 credits toward a Bachelor’s Degree in a field other than Health or Human Services may meet this educational requirement if a minimum of 20 of his/her college credits are in Health and Human Services. A transcript must be provided.
One year working with people with developmental disabilities or one year experience as a Service
Coordinator with any population required (not required for those with a Masters Degree in Health or Human Services).
Strong communication, interpersonal and problem solving skills.
Clearance through state mandated Background/Fingerprint Check(s).
