Shift schedule is 4/10s or 5/8s with alternating weekends. This is a 13 week contract with possibility of extension.The _*RN Case Manager *_will work collaboratively with the multidisciplinary healthcare team to assure the appropriate allocation of hospital resources while maintaining the quality of patient care. The _*RN Case Manager *_actively participates in the discharge planning process using recognized standards of care to determine the medical necessity, most appropriate level of care and correct patient status for service delivery.*Job Qualification** Required- Active New York RN License* Required

• Minimum of 1 years experience with hospital Case Management/Utilization Review* Required
• Minimum of 1 years experience with discharge planning and knowledge/use of InterQual.* Required
• NYS PRI Certified* Preferred
