COMMUNITY NURSE PRACTITIONER (ISNP) Manhattan, NY $130,000 – $145,000 and Excellent Benefits! $15,000 Sign-on Bonus! Field based job - territory is Manhattan Our client is a Medicare Advantage health plan whose members are living at home. Position Summary: In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self - management education & tools to better manage patient’s multiple conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned at-risk panel of Health Plan members who reside in the community. The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family, and interdisciplinary team. Responsibilities:
Performs Comprehensive Clinical Assessments in the home setting and develops plan of care utilizing evidence-based practice recommendations.
Collaborates with Plan Case Manager to schedule and follow-up home visits to update the care plan.
Ensures safe transition from setting to setting, conducts post discharge assessment within a reasonable time.

Evaluates appropriateness of member transfers for acute inpatient services,
prospectively and retrospectively and shares necessary clinical information with collaborating physicians/facilities.
Orders diagnostic services and/or therapeutic procedures as necessary to maintain member health.
Provides, or insures, that annual preventive care services are administered and properly documented in the medical record.
Optimizes pharmacy utilization through review and reconciliation of medication profile and EP formulary especially generic utilization and potential drug-drug or drug-disease interactions.
Works in continuous collaboration with primary care physician
NP will provide individualized care for the at-risk population that would improve care coordination, reduce avoidable utilization of hospitals, improve the health and quality of life of community dwelling Health Plan members.
Qualifications:
Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
One to two years clinical experience in the role of nurse practitioner required.
Minimum one year in geriatric specialty
Previous clinical RN experience in home health, acute, subacute and/or long-term care setting preferred.
Prior experience in managed care experience is preferred.
NYS RN/NP License, Registration, DEA certificates
