Essential Job Duties
- An IH&C LPN is responsible for our individual's daily care, safety, training, and supervision. These duties include but are not limited to:
- Intensive care of individuals, providing 1:1 services/support to individuals.
- Transport individuals to medical appointments, personal shopping, recreational activities
- Help individuals in and out of the vehicle.
- Socialization with other individuals
- Accurately complete all documentation and reports of individuals (progress notes, mileage logs) submit them to supervisor.
- Building relationships with families, assigned individuals, and members of the RBL BH team.
- Assist individuals with ensuring they have everything they need for the day's outing
- Collaboration with other community-based team members
- Provide for an individual's physical needs including but not limited to:
- Diaper changing, assistance when walking, feeding, bathroom needs.
- General knowledge of nursing and health care terminology.
- Excellent patient care skills.
- Knowledge of tube feeding for nutritional nourishment and medicine administration
- Postsecondary certificate from vocational school or at least an Associate's degree in Nursing required.
- Current state licensure required.
- May require prolonged periods of standing, moving, and bending.
- Must be able to lift up to 75 pounds at a time.
