Job Title: In-Home and Community Licensed Practical Nurse (IH&C LPN) Job Classification: Non-Exempt, Part-time Reports to: RBL Behavioral Health CEO Michael Bart, LSW Essential Job Duties
An IH&C LPN is responsible for our individual's daily care, safety, training, and supervision. These duties include but are not limited to:
Intensive care of individuals, providing 1:1 services/support to individuals.
Transport individuals to medical appointments, personal shopping, recreational activities
Help individuals in and out of the vehicle.
Socialization with other individuals
Accurately complete all documentation and reports of individuals (progress notes, mileage logs) submit them to supervisor.
Building relationships with families, assigned individuals, and members of the RBL BH team.
Assist individuals with ensuring they have everything they need for the day's outing
Collaboration with other community-based team members
Provide for an individual's physical needs including but not limited to:
Diaper changing, assistance when walking, feeding, bathroom needs.
Required Skills/Abilities
General knowledge of nursing and health care terminology.
Excellent patient care skills.
Knowledge of tube feeding for nutritional nourishment and medicine administration
Education And Experience
Postsecondary certificate from vocational school or at least an Associate's degree in Nursing required.
Current state licensure required.
Physical Requirements
May require prolonged periods of standing, moving, and bending.
