About the Role:

Redeemer Services Inc. seeks a dedicated, hard-working, and empowering individual to join our leadership team as a Case Management Supervisor. In this role, you will lead a team of case managers, working within the disability waiver programs in Hennepin County to enrich and support the lives of people with disabilities within their community.

Key Responsibilities:

• Advise, consult, and promote collaboration between case managers, persons served, and federal, state, and local agencies regarding waiver programs.
• Facilitate effective coordination and implementation of services.
• Enhance understanding of waiver services among case managers, persons served, and local agencies.
• Monitor employees’ caseloads, productivity, and client outcomes to ensure goals are met.
• Collaborate with case managers to improve the quality of case management activities, including planning, coordination, monitoring, referral, and linkage, and advocacy.
• Train, guide, teach, manage, and support new and current case managers to perform their jobs efficiently and meet company, DHS, and county standards.
• Prepare and conduct administrative tasks to set your case managers and teams up for success.
• Ensure that the case managers and team comply with contractual and licensing standards set by the county and state.

Qualifications:

• Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services, or a closely related field (Master’s preferred but not required), OR a Bachelor’s Degree in any field with one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
• A minimum of 2 years of proven leadership and management experience in any formal position.
• A minimum of 1 year of experience working as a Waiver Case Manager with Hennepin County.
• Strong interpersonal skills with the ability to build rapport with diverse populations.
• Excellent communication skills, both verbal and written.
• Proven ability to work independently and as part of a team.
• Ability to come into the office as needed or on-demand once qualified for a hybrid schedule.

Job Type: Full-time, non-exempt.

Hours: Monday – Friday, 8:30 am – 5:00 pm

Work Location (Hybrid): Employees will work in-office for the first 90 days. Upon successfully completing the 90 days and with permission from the Executive Director, employees will receive one day working from home, increasing up to four days by the end of the probationary and onboarding period (first 6 months or 180 days). Supervisors are expected to come into the office as needed regardless of work-from-home status.
