Redefined staffing, LLC is looking to hire a motivated and energetic Licensed Practical Nurse (LPN) to deliver remarkable resident care in an Assisted Living/ Long Term Care facility (LTC). The Licensed Practical Nurse (LPN) is responsible for providing and coordinating patient care and passing medications and providing treatments. The Licensed Practical Nurse must demonstrate the ability to make clinical judgments in an effective and efficient manner under the supervision of a Registered Nurse.
Responsibilities include but are not limited to:
Utilizing the nursing process to assess, plan, implement and evaluate resident’s care.
Performing focused interviews to identify specific needs.
Assessing signs and symptoms indicating physiologic and psychological changes in the resident’s condition.
Documenting the resident’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
Collecting samples for lab testing as assigned
Identifying and prioritizing actions based upon resident’s care requirements.
Monitoring consistency between resident’s plan of care and actions of medical staff.
Performing interventions according to identified priorities, plan of care, and the facility policies and residents outcome standard.
Providing a safe, comfortable, and therapeutic resident environment.
Initiating and evaluating residents and/or family education.
Revising the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
Documenting resident’s response to nursing and other interventions.
Communicate with patient’s family or friends to provide advice, comfort, and release instructions
Using clinical judgment in evaluation activities to meet resident’s care needs of an assigned floor/unit including establishing priorities.
Monitoring safety and effectiveness of the environment and equipment.
Participating in activities to facilitate good interpersonal communications with the chosen facility.
Using knowledge and practices in accordance with the Nurse Practice Act; Standards of Nursing Practice; Standards of Patient Care; Joint Commission, state and local laws; Patient’s Bill of Rights.
