What will I do as an employee with Redstone?

As a Health Navigator, supporting the Redstone mission will include the following responsibilities:

• Performs services of social service for assigned clients of the Skilled Nursing Facility. Social Service functions include but are not limited to: Coordination of hospice referrals; coordination of psychological and psychiatric consultations; SOS (complaint) process; calls to families of assigned long-term care residents to provide updates; attending care conferences and documenting progress notes as necessary.
• Completes assigned psycho-social assessments as scheduled. Establishes, maintains, and updates Care Plans accordingly to meet the resident's psycho-social needs.
• Completes assigned section of the MDS assessment. Reviews charting and enters data regarding client's psychosocial information, behaviors, and moods into the MDS system.
• Participates as a member of the Interdisciplinary Team (IDT) to coordinate services for assigned clients.
• Participates as a member of the Quality of Life Team to evaluate and identify any newly needed services of the long-term care residents.
• Collaborate with the Lifestyles team to assist in providing Dementia Care programming for individuals living within the Redstone Community
• Collaborate with the Nursing Team to develop interventions/plans of care for residents within our SNF living with dementia.
• Support people affected by dementia by completing holistic assessments and creating person-centered support plans based on each individual's health, wellbeing, and social support needs. This will involve visiting people in their own homes and other community settings. Ensure ongoing support is given to people affected by dementia and their caregivers empowering them to prepare for future planning and remaining as independent for as long as possible.
• Provides scheduled dementia support groups and other programming related to cognitive impairment to meet the needs of family and friends of those with dementia.
• Provide staff training for Dementia Care.

What do I need for this role with Redstone?

• Five (or more) years of related experience in a health care setting (preferred).
• Requires dementia certification or willingness to obtain certification.
• Must possess valid driver's license
