Position Summary:The Clinical Pharmacist is responsible for providing medication therapy management services in collaboration with other providers to assure that drug therapy is safe, effective, efficient and cost-effective. Pharmacy services provided include but are not limited to patient care rounds, admission and discharge medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various scholarly activities. The Pharmacist performs both clinical and distributive tasks as necessary. The Pharmacist will provide training and education to pharmacy resident, students through practical and didactic experiences. Incumbent acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Lead Pharmacist and Pharmacy Director. The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies. We provide pharmacy services in the Hospital, Specialty/General Clinics, Nursing Home and work collaboratively with our Outpatient and Ambulatory Pharmacy teams. The incumbent will be expected to handle hazardous drugs (chemotherapy) with appropriate primary and secondary engineering controls.

Job Relationships:Reports to Director of Pharmacy; collaborates with department directors, physicians, patients, families and employees of
RAMC as well as vendors, public, civic organizations, regulatory agencies and other provider organizations.

Education and Training Required:

◾ Pharmacy degree from an accredited college of pharmacy
◾ ASHP accredited pharmacy (PGY-1) residency training preferred

◾ LTC consultation experience is preferred.

◾ Working knowledge of clinic-based operations preferred.

◾ Certification by Board of Pharmacy preferred

◾ Licensed to practice in the State of Wisconsin.

Experience:◾ Minimum one (1) year hospital/community pharmacy experience.

Physical Requirements:Physical Demands:⚬ Frequent walking, standing, sitting, reaching, manual/fine dexterity, grasping, and hearing-close distance.
⚬ Occasional bending, twisting, crawling, kneeling and squatting.
⚬ Occasional ability to lift/carry or push/pull up to 50 pounds.
Sight:⚬ Near and far vision is required in order to visualize computer screens, review documents, assist patients and other
members of the healthcare team.
Cognition/Communication:⚬ Ability to participate in all aspect of communication in regards to writing, reading, math, hearing and comprehensive
speech skills.
Environmental Conditions:⚬ Work is largely performed in a pleasant, indoor hospital environment.
◾ Potential Hazards: OSHA Class 1: Class 1 HCWs, as defined by the OSHA Bloodborne Pathogen Standard, will receive additional and
specialized education on infection risks related to their employment and on preventive measure that will reduce
those risks.
⚬ Occasional exposure to infectious blood/body fluids and diseases requiring infection control practices, red infectious
waste.
⚬ Occasional exposure to needles/syringes/sharps during medication administration, assistance with treatments,
invasive procedures.
⚬ Occasional exposure to chemical compounds of medication and treatments (chemotherapy).
⚬ May handle hazardous drugs.
