Regional Hospice is expanding services and we are excited to recruit a FULL TIME Pediatric Hospice & Palliative Care RN Case Manager for our pediatric home hospice patients! Help us fulfill the need to compassionately serve as our pediatric hospice community is growing. We are looking for an experienced pediatric hospice and palliative care RN to work 40 hrs/wk and be responsible for the coordination and management of all patient care and other services for pediatric patients and their families. In this position you will work with interdisciplinary team including the patients' pediatricians in order to provide quality pain and symptom management, as well as support to the patient and family. The territory for this position will be Fairfiled County, and as our pediatric community continues to grow, we will be able to regionalize the territory more. Qualifications

• Registered Nurse with a current license in the State of Connecticut REQUIRED.
• Minimum of three (3) years' experience as a pediatric nurse in a hospital, and at least one (1) year of experience as a pediatric nurse in hospice/palliative care REQUIRED.
• Advanced credentials in pediatric hospice care (CHPPN) is preferred.
• A clean driving record and proof of auto insurance is REQUIRED.
• Excellent verbal, written and computer skills REQUIRED
• Knowledge of pediatric resources and ability to communicate effectively with families, physicians, interdisciplinary team members and others.
• Bachelor of Science in Nursing degree preferred.
