Community Health Educator
A Brief OverviewCommunity Health Educator works within the Women's Services Department to provide education to promote healthy behaviors particularly in the areas of preconception, pregnancy, and postpartum. Educational sessions will include but are not limited to health and well-being during pregnancy, tobacco prevention/cessation, obesity, physical activity, substance misuse and other areas that impact the overall health of the patient. The Community Health Educator will work as part of a team or independently with both communities and individuals. They are a crucial part of health promotion and program planning and are often the voice of public health.
What You Will Do
  • 1. Coordinates and provides education about behaviors, such as lifestyle changes, that can improve their health. Explore treatment plans based on factors like a person’s budget, schedule and health goals. a. Performs studies to assess conformance to specifications, standardization of educational materials, measures, best practice and health standards. b. Collaborates with assigned Program Manager and Leadership staff to select specific topics for review.
  • 2. Assists in establishing review criteria and standards for measuring and monitoring educational materials, services, patient improvement as well as ensuring compliance with grant requirements.
  • 3. Utilizes analytic skills and statistical analysis to aggregate data to assist with the identification of community based education patterns and trends to establish priorities and recommend improvement activities.
  • 4. Provides administrative support for special projects and studies; collects and compiles data, performs statistical analysis and follows-up as requested or needed.
  • 5. Prepares and maintains required reports, records and files for operational, administrative and compliance purposes. Utilizes computer and information systems to enter, format and retrieve data, generate statistics, computations, tables, charts and graphs.
  • 6. Assesses the environment, personnel, internal operations, and processes to determine approaches and practices that will coordinate group sessions, equipment, and materials required to achieve quality and efficient educational sessions.
  • 7. Attends staff in-services, training and orientation on community-based education; as well as participates as needed in the Community Consortium.
  • 8. Maintains professional credentials and education necessary to keep abreast and maintain a current knowledge of educational/theoretical/performance improvement principles, concepts and techniques, best practices and current health education trends. Attends continuing education, staff development, and mandatory programs.
  • Additional responsibilitites include:
  • Facilitating community based education group session
  • Educating women in the preconception, pregnancy, and postpartum phases in a community based group setting and individually
  • Initiating community activities
  • Program development, implementation, and evaluation
  • Working with Program Manager and Consortium Coordinator to communicate information to community partners
Qualifications
  • Bachelor's Degree in Health Education or a related field Required
  • Additional experience and skills may be required based on area of assignment.
  • At least 1 year experience Community Health Education experience with comprehensive knowledge of facilitating curriculum based education Preferred
