Patient Navigator - Ambulatory A Brief OverviewThe Patient Navigator serves as a consistent care coordinator throughout the continuum of care for patients and families throughout their healthcare journey. A patient navigator helps patients communicate with their healthcare providers so they get the information they need to make decisions about their health care. Coordinates communication with providers and other healthcare team members on behalf of the patient without close supervision. What You Will Do
Creates a seamless flow through the health system to facilitate the patient’s care.
Facilitates improved health care access and quality for underserved populations through advocacy and care coordination
Offers prenatal-specific health education, support, and connect patients to pediatric care, new mother's resources, and community support
Organizes schedules and manages appointments for patients to ensure they receive services in a timely manner
Facilitates communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
Manages health care records
Informs the patient on financial aspects of care and linking patient to financial resources as needed.
Proactively manages relationships with patients.
Accommodates interpretive language services when needed
Manages follow up visits and ensuring continuation of care and linkage to community resources
Works Collaboratively with all members of the healthcare team to meet the patient needs.
Identifies and communicates any social challenges faced by the patient.
Maintain visibility and accessibility to assigned patient base.
Effectively manage time, workload and resources.
Qualifications
Successful completion of an approved Medical Assistant program. Required
Certified Medical Assistant (CMA)-AAMA Upon Hire Required and
Patient Navigator Certification Upon Hire Preferred
Minimum 2 years experience of OB/GYN outpatient office or related experience Required and
Hospital experience Preferred
