Registered Nurse, Oncology Navigator A Brief Overview The Registered Nurse, Oncology Navigator serves as a consistent care coordinator throughout the continuum of care for patients and families throughout their healthcare journey with Regional One Health. Uses critical thinking to assess the physical, emotional, and social needs of the patient, and improves the overall health, treatment, and/or end-of-life outcomes. Responsible for facilitating informed decision making and timely access to quality care in an effort to decrease barriers and improve patient experience. Coordinates communication with providers and other healthcare team members on behalf of the patient without close supervision. Meets the needs of the patient in relation to education, screening, prevention and follow-up care.What you will do
Employs a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocating for options and services to meet an individual’s health needs through communication and available resources to promote quality cost effective outcomes.
Creates a seamless flow through the health system to facilitate the patient’s care.
Decrease healthcare barriers by assessing the needs of patients, families, and caregivers and providing anticipatory guidance and education.
Develops and coordinates education, resources, and activities that provide understanding and support for patients and families without close supervision.
Maintains reliable systems to document, track and monitor patient data.
Track patients’ progress and outcomes while collaborating with oncologist to adjust plans as needed.
Schedules appointments with the patients, converse with providers and specialists, contact outside agencies and companies for their patients, and/or arrange other services that will enhance the patient’s care and well-being.
Assists patients in managing their health conditions, prioritizing their healthcare needs, and preventing complications through standardize care protocols.
Working with insurance coverage, community resources, integrative oncology, and other supportive services to enhance the patient’s care and wellbeing.
Educates patients on general preventative care, symptom management, and individualized plan of care to prevent unnecessary admissions, readmissions, and over-utilization of resources.
Performs any other duties required to manage the patients care that is within the scope of nursing science and clinical training.
Qualifications
Bachelor's Degree in Nursing or Graduate of from an accredited nursing program with ten (10) years of experience. Required
RN - Registered Nurse - State Licensure and/or Compact State Licensure Current license to practice as a registered nurse in the state where work is performed Upon Hire Required and
OCN - Oncology Nurse Certification within 1 Year Required
Minimum 2 years experience of nursing experience in an oncology setting Required and
Outpatient nursing and scheduling experience Preferred
