Master of Social Work (MSW) Job description:

Rehab Accomplished is a therapist-owned staffing agency serving Kaufman, TX and surrounding areas. We provide physical therapy, occupational therapy, speech therapy, and social work services. Our mission is to provide quality, evidence-based intervention and make a difference for others.

We are seeking a dedicated and compassionate Master of Social Work (MSW) to join our team. As an MSW in healthcare, you will play a pivotal role in providing comprehensive support to patients, their families, and the healthcare team. Your expertise in social work will be instrumental in ensuring the well-being and emotional health of our patients throughout their healthcare journey.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. This list is not to be construed as exclusive or all-inclusive.

• Offer emotional support and counseling to patients and families, helping them cope with the challenges of medical conditions and hospitalization. Act as a patient advocate, ensuring their needs and preferences are addressed within the healthcare system.
• Collaborate with the healthcare team to create individualized care plans, ensuring seamless transitions between different levels of care and healthcare facilities.
• Conduct thorough psychosocial assessments to identify social and emotional factors influencing patients' health and well-being. Utilize these assessments to develop effective intervention plans.
• Provide immediate support and crisis intervention during emergencies or critical situations, assessing mental health, safety, and risk factors to ensure patient well-being.
• Assist in discharge planning, evaluating patients' social support systems, home environment, and access to resources to ensure a safe and successful transition back home or to another care setting.
• Specialize in providing counseling, support groups, and treatment referrals for patients facing addiction or mental health challenges.
• Offer information and referrals to community resources and social services to address patients' needs for financial assistance, housing support, transportation, and other essential resources.
• Provide emotional and psychosocial support to patients with life-limiting illnesses and their families, addressing end-of-life issues and offering grief and bereavement support.
• Participate in ethics consultations, providing insights and guidance on complex ethical issues related to patient care and medical decision-making.
• Contribute to research projects and program development initiatives focused on improving healthcare services and addressing social determinants of health.

Qualifications/Skills:

• Proven experience of 1 year in a healthcare or medical setting, preferably with a focus on patient care and support. New Grads are also encouraged to apply
• Master's degree in Social Work (MSW) from an accredited institution
• Licensed in the State of Texas
• Excellent communication and interpersonal skills to build meaningful connections with patients and healthcare professionals
• Ability to work effectively in a team-oriented environment and collaborate with multidisciplinary teams
