Rehabilitation Support Services in Chenango County is looking for a Transitional Engagement Specialist to join our team. Monday to Friday 8am -5pm. Maintain a caseload, conduct outreach visits, advocate for clients, keep detailed records, collaborate with internal and external stakeholders, provide coordination of care, work independently to prioritize tasks, and other duties as described in the Job Description. Bachelor’s degree in a relevant field preferred – SW, Psych Counseling and at least one year of experience in a similar role. Excellent benefit package offered.
