Rejoice DDS is Hiring a Program Coordinator Near Omaha, NE

Position Purpose

Initiate new participants to our day and residential programs, provide on-going monitoring of staff and participants, and ensure the quality of services provided for adults with intellectual and developmental disabilities.

Job Duties

1. Communicates with the individual to determine services needs/preferences, service options, etc., and ensure eligibility/authorization for funding of services/supports is available.

2. Involves the individual and his/her community and natural supports to develop a comprehensive, holistic and person-centered strategy to address issues and concerns as they arise.

3. Identifies all services/supports and, together with the agency team, develops a service plan involving agency and external resources.

4. Gathers, interprets and operationalizes information about the individual and makes available to the agency’s team. Provides all required notifications to the individual and assists with the filing of an appeal, if applicable.

5. Attends team meetings, and obtains all necessary consents, completes a social history and other agency-required forms/paperwork, records and documentation.

6. Facilitates the completion of required assessments, services and safety plans, and coordinates with the individual’s team to develop service, medical and behavioral support protocols.

7. Monitors to ensure that significant changes in the individual’s needs are addressed immediately, that the service plan is revised, and that gaps in services are addressed by the agency team.

8. Supports the individual to evaluate and determine the effectiveness, appropriateness and quality of his/her services/supports.

9. Maintains individual and agency records and ensures all electronic and physical records contain current information.

10. Identifies and coordinates the individuals’ clinical and service needs and ensures that there is coordinated plan of care with agency and external agencies/professionals.

11. Communicates with the agency team regarding health concerns and takes appropriate action.

12. Completes monthly program reports.

13. Provides training and mentoring to staff in the provision of supports for community integration.

14. Serves as a member of agency committees and workgroups, and as a liaison to external agencies/professionals.

15. As part of the agency’s management team, provides input into the development of individual service plans and implements policy and procedures.

16. Assure programs are in compliance with state and federal regulations and Rejoice quality expectations

17. Represents Rejoice, at periodic meetings.

Requirements/Qualifications

  • PREFERRED: Experience in program writing, program implementation, assessments, team facilitation, supervision of the work of others, experience with working with individuals with disabilities. Experience in Word, Excel, Outlook, and Therap.

· Knowledge of: Current practices in the field of community-based services for persons with intellectual disabilities and other services for persons with developmental disabilities

· Person-centered program planning

Job Types: Full-time, Part-time

Pay: $45,000.00 per year

Expected hours: 40 per week
