Job Duties:

• Conduct home visits with clients for start of cares, recertifications, resumptions, and supervisory visits.
• Complete appropriate documentation for visits, such as: assessments, Plan of Care, Aide Plan of Care, and supervisory visits.
• Provide routine visits as needed to clients as directed by their service plan.

Requirements:

• Current state nursing licenses.
• Current CPR certification (BLS)
• Reliable transportation and valid car insurance
• Ability to work as part of a team and communicate effectively with staff, families, and visitors.

Pay: From $35.00 per hour

Expected hours: 30 – 40 per week
