Overview:
We are a cornerstone in community health care, serving vulnerable populations including at-risk children, the elderly, and those with chronic illnesses. We are currently seeking a full-time Physician to join our team, providing high-quality care to homebound patients and their families NJ.

Qualifications:

• Doctorate degree
• Minimum of 2 years of experience in Primary Care
• Valid Drug Enforcement Administration (DEA) and Controlled Dangerous Substances (CDS) certifications
• American Heart Association Basic Life Support (AHA BLS) certification
• Board certification in Family Medicine or Internal Medicine in NJ

Preferred:

• Minimum of 1 year of experience in Geriatric Care, Senior Care, or Geriatric/LTC
• National Provider Identifier (NPI) Number

Role Overview:
As a Physician with our organization, you will play a pivotal role in delivering exceptional medical services to homebound patients, focusing on symptom management and enhancing quality of life. The ideal candidate will have a strong background in primary care settings, with experience in geriatrics recommended.

Key Responsibilities:

• Provide direct medical services and consultation to primary care homebound patients
• Monitor patient's condition and responsiveness to treatment
• Maintain complete patient records using Electronic Medical Record systems
• Ensure compliance with regulatory requirements and agency policies
• Collaborate in education programs for staff and participate in community outreach initiatives
• Adhere to infection control and safety policies

Schedule and Shift Details:

• Full-time position, 8:30 am - 4:30 pm
• 1 weekend per month
• Rotation for summer and winter holidays
