Renew Counseling Center of N.C is Hiring a TCM Care Manager Supervisor Near Fayetteville, NC

This position falls under the Alliance Health Tailored Plan.

The Care Manager Supervisor is responsible for the successful provision of Tailored Care Management services to clients with complex behavioral health and healthcare needs. Clinical, administrative and operational supervision will be provided to Care Managers by rendering support and guidance to coordinate care and services needed to assist in achieving optimal health, wellness and life goals. There will be direct oversight of Care Managers to ensure provision of high-quality care management, including care management comprehensive assessment and reassessment, care coordination, health promotion, comprehensive transitional care, individual and family support, referral to community and social support services, and use of Health Information Technology. This position is a dynamic, proactive approach to supervision, assessment, monitoring and comprehensive management of care.

Qualifications:

NOTE: Renew Counseling Center of NC will serve members with behavioral health conditions; therefore, staff member must meet BOTH of the following criteria:

Have one of the following credentials:

  • Licensure (full or associate-level), certification, registration, or permit issued by the governing board regulating a human service profession (including Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA)

OR

  • Registered Nurse (RN) license issued by the North Carolina Board of Nursing

AND

Three (3) years of experience providing care management, case management, or care coordination to the population being served*.

*Population being Served – the disability group behavioral health (B/H) and age of individuals served.

Job Roles, Duties and Responsibilities

  • Supervision of Care Managers: One supervising care manager will not oversee more than eight (8) care managers. Supervisors should have no caseload, but will provide coverage for vacation, sick leave, and staff turnovers;
  • Oversight of specific care management functions: Ensure care management staff are meeting expectations, requirements and timelines in these areas: Care Management Comprehensive Assessment; care plans; development of care team; care coordination; twenty-four (24) hour coverage; annual physical exam; health promotion; addressing unmet health-related resource needs; transitional care management; diversion; and ensuring the minimum contact requirements are met based on the client’s acuity;
  • Provide oversight and management to the daily operations of care management; review and approve progress notes/encounter data and other documentation; review and analyze data, reports, and trends in the EHR and Care Management Systems;
  • Reviews all Tailored Care Billing for accuracy prior to processing;
  • Development of positive working relationships with Clinical Consultants;
  • Ensures that clients are assigned and risks stratified by care management staff;
  • Ensures all operational, administrative, and compliance functions within the agency are being properly executed in accordance with regulatory-based best practices;
  • Assists with clinical needs within the agency as it relates to client care, as needed;
  • Provides clinical support to tailored care management staff and clients, as needed;
  • Participates in continuous performance improvement;
  • Adheres to the Tailored Care Management Provider Manual;
  • Responsible for ensuring that all data requirements are met by adhering to established IT protocols;
  • Oversees safe and risk-free environment according to policies and guidelines as applicable by local, state, federal and accrediting agencies;
  • Is a role model for staff by adhering to the stated mission, vision, and values of the agency;
  • Assists with promoting collaboration for the client’s continuum of care with all clients;
  • Provides monthly supervisions to Care Manager(s);
  • Participates in case reviews, Child and Family Team Meetings and Adult Treatment Team Meetings (when necessary and to assist care management team);
  • Provides appropriate documentation in the client’s EHR; NOTE: Must adhere to the agency’s internal documentation protocols and NC DHHS Records Management and Documentation Manual APSM-45-2;
  • Abiding by agency’s guidelines, policies and HIPAA regulations;
  • Reporting client incident reports;
  • Participates in monthly supervision from designated supervisor;
  • Responding to emails and TEAMS chat within a timely manner;
  • Participates in scheduled staff meetings;
  • Participates in scheduled QA/QI Committee meetings; and
  • All other duties as assigned.

Required Care Management Trainings (stipulated by NC DHHS)

This position must undergo and complete the full Tailored Care Management intensive training curriculum, regardless of previous experience. Trainings must include practical (“hands-on”) training modalities and evaluation, which may include role play, use of call scripts and practice sessions. The on-line self-pace courses are sponsored by the Area Health Education Center (AHEC). Training certificates will demonstrate competency. Care management services cannot be rendered without receiving trainings in the following domains: NOTE: There will be multiple training topics that will fall under the below domains.

  • Tailored Plan eligibility and services
  • Whole-person health and unmet resource needs
  • Community Integration
  • Components of Health Home/Tailored Care Management
  • Ethics, boundaries, and personal safety
  • Building a trusting relationship
  • Health promotion
  • Transitional care management
  • Skills to support health behavior change, including motivational interviewing
  • Person-centered needs assessment and care planning, including LTSS and other needs
  • Preparing members for and assisting them during emergencies and natural disasters
  • Infection control and prevention practices
  • Understanding virtual/telehealth applications
  • Needs of justice-involved individuals
  • Additional trainings for serving:
  • Children
  • Children with complex needs
  • Pregnant and postpartum women with substance use disorder (SUD) or SUD history
  • Members with Long-Term Services and Supports (LTSS) needs.

  • Client Rights (within 30 days of employment)
  • HIPAA and Confidentiality (within 30 days of employment)
  • Alternative to Restrictive Interventions - NCI Part A (exempted; however must sign Attestation for personnel file maintenance)
  • Infectious Diseases and Bloodborne Pathogens (within 30 days of employment)
  • Other designated trainings required by the agency and CARF to enhance job knowledge and performance
