RevCore Recovery Center of Manhattan, LLC is seeking Social Worker or Mental Health Counselor for the outpatient mental health program, located in Brooklyn. This position is an excellent opportunity for clinicians to develop their skills in the areas of assessment and treatment planning, psychotherapy, crisis intervention, complex care management and discharge planning. These services promote hope and recovery, and they will focus on client choice and achievable outcomes.
These services will be delivered primarily in the offices of the program but will be provided in the client’s natural environment in crisis situations, and when engagement and outreach services are being provided.
Assessment and Treatment Planning Clinicians will provide assessment and treatment planning services in partnership with each client on an ongoing basis. A complete and thorough assessment of both the mental illness, as well any co-occurring substance use disorder, will be accomplished. The outcome of this service will be much more than arriving at a DSM-V diagnosis. The outcome will be the development of a comprehensive, individualized, culturally sensitive, goal-oriented treatment plan. It will identify the mental illness, the symptoms and the effects on the person’s ability to function in major life roles.

Family Psychoeducation/Psychoeducation
Wellness Self-Management
