Revolutionary Home Health is a Medicare-certified agency proudly servicing patients throughout Pennsylvania! We pride ourselves on our exceptional team of nurses that give the best care to our home health patients in the community! We are seeking dedicated and compassionate nurses to join our team of healthcare professionals!
Our orientation consists of a 6-week training program combining time in a classroom setting focusing on learning rules and regulations of home health, hands-on skills, and documentation guidelines, and then time spent in the field shadowing a preceptor and applying your knowledge caring for patients.
Job Duties Include:
· Performing in-home visits focused on patient assessment, teaching, and care
· Completing comprehensive patient assessments and developing patient plans of care
· Performing patient care including wound care, lab draws, and IV administration
· Completing documentation in a timely manner and communicating as necessary with patient physicians and other healthcare providers
· On-Call Rotation for Weekdays/Weekends/Holidays
· Participating in Multidisciplinary Team Meetings

Qualifications:
· Graduate of an accredited school of nursing
· Current RN license to practice in Pennsylvania
· Valid PA Driver’s License
· Hospice and/or Home Health experience a PLUS, but willing to train committed candidates
