Randstad Healthcare is hiring a Licensed Social Worker in the Philadelphia area. This is a contract position with the possibility of extension or permanente placement.

Details:

• Position is Remote but MUST LIVE IN PHILADELPHIA, PA area.
• Schedule: 8-5 EST M-F

Qualifications:

• Must have Masters degree and LSW or LCSW in PA
• Ideally 3 years of experience in Medical Social Work doing case management in Hospital, care facilities, or home care. Ideally with Discharge planning.
• Bilingual Spanish is a PLUS but not required

Responsibilities:

• Call center setting supporting outreach to patients
• Making calls to patients that are behind on wellness visits to primary care, behind on prescriptions or other key care gap items
• Will be setting patients up with PCP, and outside resources as needed (meals on wheels, home health aids, transportation etc.)
• Main communication with patients will be via phone call and mail outs to those they can not reach by phone

Job Types: Full-time, Contract

Pay: $31.00 - $32.00 per hour
