- Performs RN duties in accordance with our policy and procedures for all programs that are assigned.
- Completes and participates in the assessment of patient needs and implementation of patient care.
- Participates in patient care planning meetings.
- Notifies and documents change in condition of client(s).
- Makes periodic rounds and/or calls to each assigned patient.
- Visit's client's homes to assess the needs for new patients.
- Develops relationships with referral sources to assist in getting patients open for services.
- Do supervisory visits to client's homes as required.
- Registered Nurse license in the state of WV
- Associate's Degree
- Excellent customer service skills
- Ability to work independently
- Must be able to develop relationships with case managers and social workers at the hospitals. Travel will include covering Raleigh and Fayette counties.
