Right at Home has a rewarding position available for the right self-motivated individual in Naples. Candidate will be responsible for ensuring clients receive the highest quality care.
Essential Functions:
*Evaluate and supervise all field staff to ensure all policies and procedures are being followed.
*Communicate with all parties involved in the client's care: Family, GCM, Hospice, etc.
*Ensuring all clients are receiving top quality care.
*Perform and supervise case management activities; including developing and updating care plans.
*Visit every client on a monthly basis and updating the primary guardian afterwards
Requirements:
*Strong experience in case management and execution, creative, outstanding communication skills and have a pleasant outgoing attitude. Local driving required.
