Role Description

This is a full-time hybrid role for a Case Planner at Rising Ground in Brooklyn, NY, with some flexibility for remote work. The Case Planner will be responsible for providing caring support and proven pathways for positive change to individuals and families. Day-to-day tasks include conducting assessments, developing and implementing case plans, coordinating services, and advocating for clients' needs.

Qualifications

• Case management, social work, and counseling skills
• Knowledge of community resources and support services
• Strong communication and interpersonal skills
• Ability to work collaboratively in a team setting
• Experience in child welfare or family services
• Bachelor's or Master's degree in Social Work, Psychology, or related field
