About River Edge Behavioral Health
Serving more than 10,000 Georgians, River Edge continues to be one of the state's leading resources for mental health, substance use disorder treatments and supports as well as individuals with intellectual and development disabilities. River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups.
River Edge is currently seeking a Full-time Case Manager for Permanent Supportive Housing. We’re seeking a vibrant candidate who has a passion for helping individuals experiencing housing instability, as well as brainstorming potential partnerships or methods for engagement with the local community. The initial goal is to assist adult individuals with securing and maintaining stable housing.

Duties and Responsibilities
  • Providing intensive housing support coordination & individual training for the adult homeless population.
  • Working closely with housing units/complexes, private landlords, community partners/agencies, and other Housing programs.
  • Connecting homeless individuals to housing supports/ resources as well as other resources identified as being needed such as mental health, case management, skills.
  • Assisting with management of the permanent housing units
  • Participate in community outreach to individuals experiencing homelessness or housing instability.
  • Assisting individuals with the application process for various housing subsidies/programs.
  • Conduct a minimum of 1 home visit per week/per client within their community setting or within client’s current residence.
  • Perform information and duties, such as overseeing mailings, organizing travel, monitoring budget information, and reporting on financial issues.
  • Interview client, family members, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
Qualifications
  • Bachelor's degree from an accredited college or university in a related field
  • One (1) year of experience specifically working with adults with severe and persistent mental illness and addictive disease.
  • At least one (1) year of experience working within housing coordination.
  • At least one (1) year of experience providing in-home/community services - Knowledge of community resources within metro counties.
  • Exemplary verbal and written communication skills
  • Valid driver's license with clean motor vehicle report.
  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
  • Provide 2 Professional reference Statements (Former employers only, no family members)
  • Demonstrate excellent communications skills, both verbal and written.

Client Engagement and Needs Assessment: Proficiency in engaging with clients, conducting thorough assessments, and identifying individual needs.
Resource Connection and Case Management: Skill in connecting individuals to relevant resources and providing ongoing case management.
Administrative and Organizational Skills: Strong administrative skills to manage housing units and handle various organizational duties.
Community Outreach and Relationship Building: Effective in community outreach and building relationships with stakeholders.
