Description

Position Title: Continuous Improvement Facilitator

Department: Medical Group

Reports To: VP Of Operations / COO

Position Summary: The Continuous Improvement Facilitator will be responsible for leading and managing process improvement initiatives within the family practice to enhance patient care, operational efficiency, and staff satisfaction. This role involves collaborating with clinical and administrative teams to identify areas for improvement, develop strategies, and implement solutions using methodologies such as Lean, Six Sigma, and other quality improvement frameworks.

Requirements

Key Responsibilities:

  1. Process Improvement:
    • Identify and analyze key processes within the family practice that require improvement.
    • Develop and implement process improvement plans using Lean, Six Sigma, and other relevant methodologies.
    • Facilitate process mapping, root cause analysis, and workflow redesign sessions.
    • Monitor and evaluate the effectiveness of improvement initiatives.
  2. Project Management:
    • Lead and manage multiple improvement projects simultaneously.
    • Develop project plans, timelines, and deliverables.
    • Track progress, manage risks, and ensure projects are completed on time and within scope.
    • Prepare and present project updates to senior leadership and stakeholders.
  3. Data Analysis and Reporting:
    • Collect and analyze data to identify trends, measure performance, and assess the impact of improvement initiatives.
    • Develop and maintain dashboards and performance reports.
    • Provide data-driven recommendations to inform decision-making and policy development.
  4. Training and Development:
    • Conduct training sessions and workshops to educate staff on continuous improvement methodologies and tools.
    • Mentor and coach staff on the application of improvement techniques.
    • Promote a culture of continuous improvement and innovation within the practice.
  5. Stakeholder Engagement:
    • Collaborate with physicians, nurses, administrative staff, and other stakeholders to understand their needs and perspectives.
    • Facilitate communication and build consensus among diverse teams.
    • Engage patients and families in the improvement process to ensure patient-centered care.
  6. Quality Assurance:
    • Ensure compliance with relevant healthcare regulations, standards, and best practices.
    • Conduct regular audits and assessments to identify areas for improvement.
    • Implement corrective actions and monitor their effectiveness.

Qualifications:

  • Minimum of 2 years of experience in continuous improvement or a related role.
  • High school diploma or equivalent with strong problem-solving and analytical skills, or a bachelor’s degree in Business Administration, or a related field.
  • Certification in Lean, Six Sigma (Green Belt or Black Belt), or other quality improvement methodologies is highly desirable.
  • Minimum of 3-5 years of experience in process improvement, project management, or a related role within a healthcare setting.
  • Strong analytical skills with the ability to interpret data and make data-driven decisions.
  • Excellent communication and interpersonal skills, with the ability to engage and motivate diverse teams.
  • Proficiency in process mapping, root cause analysis, and quality improvement tools.
  • Ability to manage multiple projects and priorities in a fast-paced environment.
